Updating Your Personal Details



It is vital that the practice has an up to date record of your contact details, so we can contact you about your health care and treatment.  Therefore, please contact us as soon as possible if your address, landline or mobile telephone number changes.

If you move to a new address outside of our practice catchment area of L4/L5/L6 we will advise that you must register at an alternative practice. This is because the local teams we work with such as the district nursing service will be different and also because the GPs will not have the time to travel to areas outside the practice boundary.

Unfortunately, there are no exceptions to this and you will be given 30 days to make alternative arrangements.

The NHS and the practice both are investing considerable resources in Information Technology to server the patient better and provide quality care. In line with the above, practice uses SMS/ text service to send reminder for appointment, reminder for reviews of long term condition, information about flu campaign etc., To ensure that you receive SMS/text message on regular basis, please update your contact details as soon as possible.
